A Chinook flying from Shell's Brent field to Sumburgh heliport on Shetland suffered gearbox failure which resulted in the desynchronisation and collision of the Chinook's twin rotors. The Chinook was approximately 1.5 miles from landing when it crashed into the North Sea with the deaths of 45 out of 47 crew and passengers on board (according to H&SE report. Other reports cite 44 fatalities out of 46).
